    Glioblastoma (GBM), the most aggressive form of primary brain tumor in adults, is characterized by rapid proliferation, diffuse infiltration, and strong resistance to standard therapies. Despite advances in surgery, radiation, and chemotherapy, the prognosis for GBM remains poor. As a result, there is a critical need to identify novel biomarkers and therapeutic targets that reflect the biological complexity of GBM. Exosomes, nano-sized extracellular vesicles secreted by all cell types, including tumor cells, have emerged as central mediators of intercellular communication in the GBM tumor microenvironment. They carry a diverse cargo of proteins, lipids, DNA, and various RNA species (mRNA, miRNA, lncRNA), playing essential roles in tumor growth, angiogenesis, immune suppression, and treatment resistance. Exosomes derived from glioblastoma cells can cross the blood–brain barrier, making them ideal candidates for non-invasive liquid biopsy and real-time disease monitoring.
