5S-hydroxy-6R-(S-cysteinyl)-7E,9E,11Z,14Z-eicosatetraenoic acid, commonly known as leukotriene E4 (LTE4), is a key inflammatory mediator belonging to the cysteinyl leukotriene family. It is produced by several types of white blood cells, including eosinophils, mast cells, tissue macrophages, and basophils. Recent research has also revealed that platelets adhering to neutrophils can produce LTE4. Formed through the sequential metabolism of leukotriene C4 (LTC4) and leukotriene D4 (LTD4), LTE4 is the most stable and long-lasting of the cysteinyl leukotrienes, making it the dominant form found in biologic fluids. Due to its stability, LTE4 accumulates in various biological matrices, including breath condensation, plasma, and urine, and is frequently monitored in clinical research. Elevated levels of LTE4 have been strongly associated with respiratory diseases, particularly in patients suffering from severe asthma attacks and those with aspirin-exacerbated respiratory disease (AERD). The ability to accurately measure LTE4 in biological samples, particularly in urine, is crucial for understanding its role in these diseases and for monitoring disease progression.
