Deoxycorticosterone (DOC) is a hormone secreted by the adrenal cortex, typically synthesized from progesterone through the catalytic action of the enzyme 21-hydroxylase (CYP21A2). Its chemical structure is C₂₁H₃₀O₃, characterized by a typical steroid backbone consisting of four carbon rings (A, B, C, D rings). The structure of DOC includes an aldehyde group (-CH₂OH) at the 21st carbon position and a ketone group (-C=O) at the 3rd position. Like other steroid hormones, it has a double bond between the 4th and 5th carbons in the A ring. DOC is an important intermediate in the aldosterone synthesis pathway and plays a role in regulating water and electrolyte balance in the body. The levels of DOC are regulated by the renin-angiotensin system, with its synthesis increasing especially in conditions of fluid deficiency or low blood pressure. The metabolism of deoxycorticosterone primarily involves the enzyme 11β-hydroxylase (CYP11B1), which catalyzes the production of more active compounds such as 11-deoxycortisol and aldosterone. Aldosterone, a key member of the mineralocorticoid hormone family, has stronger effects on sodium reabsorption and water metabolism regulation. In animal models and human samples, the concentration of DOC can serve as a marker for studying mineral metabolism, adrenal cortex function, and blood pressure regulation. Changes in DOC levels provide crucial insights into the mechanisms underlying diseases, particularly in the research of hypertension and electrolyte imbalances.
