Cortexolone (also known as 11-deoxycortisol, chemical formula C₂₁H₃₀O₄) is a steroid compound that serves as a precursor to glucocorticoids. Its structure, like other members of the steroid hormone family, consists of a steroid nucleus made up of four carbon rings (A, B, C, D rings). The main feature of cortexolone is the absence of a hydroxyl group (-OH) at the 11th carbon, hence the name 11-deoxycortisol. While cortexolone itself has relatively low biological activity, it is a critical precursor in the cortisol synthesis pathway. In certain experimental studies, cortexolone has been found to possess some anti-androgenic properties, particularly in dermatological applications. It can inhibit 5α-reductase activity, preventing the conversion of testosterone to dihydrotestosterone (DHT), thereby reducing the occurrence of DHT-induced skin conditions such as acne and hair loss. Cortexolone is metabolized by 11β-hydroxylase (CYP11B1) to produce cortisol, a process that occurs in the adrenal glands. Cortisol is a glucocorticoid with potent anti-inflammatory and immunomodulatory effects, playing a vital role in regulating numerous physiological processes in the body.
