9-oxo-11α,15S-dihydroxy-prost-13E-en-1-oic acid, commonly known as Prostaglandin E1 (PGE1), is a biologically active metabolite derived from the cyclooxygenase (COX) metabolism of dihomo-γ-linolenic acid (DGLA). In normal human or animal plasma, PGE1 is generally undetectable due to its rapid metabolism, yet it plays significant physiological roles. PGE1 is one of the major prostaglandins found in human semen, and its pharmacological effects include vasodilation, blood pressure reduction, and inhibition of platelet aggregation. Clinically, PGE1 is often used as a vasodilator and platelet aggregation inhibitor.
PGE1 exerts multiple therapeutic effects. It can inhibit ADP-induced human platelet aggregation, demonstrating its strong anti-platelet properties. Its vasodilatory and antihypertensive actions make it useful in treating male sexual dysfunction and as an adjunct to cardiac drugs in emergency neonatal care. PGE1 is used therapeutically for maintaining the patency of the ductus arteriosus in newborns with congenital heart defects. In severe cases of limb ischemia, PGE1 is applied to stimulate angiogenesis by dilating blood vessels and promoting blood flow to the affected areas, providing critical support in vascular regeneration. Additionally, PGE1 influences various cellular signaling pathways, including protein kinase C (PKC), adenylate cyclase activity, and calcium ion mobilization, contributing to its wide range of physiological effects.
